A CASE REPORT : TINDAKAN IMPLANT REVISION PADA IMPLANT FAILURE 1/3 MIDLE LEFT TIBIA DAN FIBULA PASCA TINDAKAN OPEN REDUCTION AND INTERNAL FIXATION PLATE AND SCREW

Implant failure merupakan salah satu komplikasi pasca Open Reduction and Internal Fixation (ORIF) yang dapat menyebabkan instabilitas fraktur hingga non-union, terutama pada regio cruris yang sering mengalami cedera akibat trauma. Insidensi fraktur tibia masih tinggi baik secara global maupun di Indonesia, dengan kecelakaan lalu lintas dan jatuh sebagai penyebab utama. Faktor yang berperan dalam implant failure meliputi kualitas tulang yang buruk, kegagalan biomekanik implan, infeksi, serta ketidakpatuhan terhadap rehabilitasi, sehingga penatalaksanaan yang tepat dan multidisiplin diperlukan untuk mengembalikan stabilitas serta fungsi ekstremitas. Dilaporkan kasus seorang perempuan 29 tahun yang datang ke poli ortopedi RSP Ibnu Sina dengan keluhan nyeri tungkai kiri bawah pasca trauma jatuh, disertai riwayat ORIF tibia sebelumnya. Pemeriksaan klinis menunjukkan deformitas dan nyeri regio cruris sinistra dengan status neurovaskular distal (NVD) baik. Pemeriksaan laboratorium menunjukkan leukositosis ringan, dengan kadar hemoglobin dan trombosit dalam batas normal. Radiografi memperlihatkan fraktur tibia 1/3 tengah dengan plate dan screw yang mengalami pergeseran minimal, celah fraktur persisten, tanpa pembentukan kalus, mengarah pada implant failure dan risiko non-union. Diagnosis ditegakkan sebagai implant revision akibat implant failure pada tibia dan fibula sinistra pasca ORIF. Pasien menjalani tindakan ORIF revisi dengan hasil posisi implan stabil, alignment tulang membaik, serta perbaikan nyeri dan fungsi tanpa komplikasi akut. Rehabilitasi pascaoperasi yang adekuat berperan penting dalam keberhasilan penyembuhan fraktur.References
