DEPRESI BERAT TANPA GEJALA PSIKOTIK DENGAN TENTAMEN SUICIDE DAN GANGGUAN KEPRIBADIAN EMOSIONAL TIPE BORDERLINE

Perilaku bunuh diri pada remaja merupakan masalah kesehatan mental yang serius dan terus meningkat. Depresi berat dan gangguan kepribadian emosional tidak stabil tipe borderline berperan besar dalam munculnya ide serta percobaan bunuh diri berulang. Trauma masa kanak-kanak, konflik keluarga, dan rendahnya dukungan sosial sering memperburuk kondisi klinis. Laporan kasus ini menggambarkan seorang remaja perempuan berusia 14 tahun 8 bulan dengan riwayat percobaan bunuh diri berulang dengan konsumsi obat, self-harm, dan perilaku membahayakan diri lainnya, disertai penolakan pengobatan psikiatri. Pemeriksaan status mental menunjukkan suasana hati depresif, afek depresif serasi dengan isi pembicaraan, ide bunuh diri, kontrol impuls buruk, serta gangguan hubungan interpersonal. Pada pemeriksaan fisik ditemukan jaringan parut pada kedua lengan dan tungkai atas akibat self-harm. Diagnosis ditegakkan sebagai Episode Depresif Berat tanpa gejala psikotik (F32.2) dengan komorbid Gangguan Kepribadian Emosional Tidak Stabil tipe borderline (F60.3), disertai gangguan fungsi sosial berat. Penanganan melibatkan kombinasi farmakoterapi, termasuk antidepresan SSRI, mood stabilizer, dan antipsikotik atipikal, serta pendekatan non-farmakologi yang dapat disarankan pada pasien ialah Dialectical Behavior Therapy, terapi keluarga dan intervensi sosial-pendidikan. Pendekatan multidisipliner ini bertujuan menurunkan risiko kekambuhan, meningkatkan regulasi emosi, serta memperbaiki fungsi sosial pasien. Kasus ini menegaskan bahwa perilaku bunuh diri berulang pada remaja merupakan hasil interaksi kompleks antara faktor biologis, psikologis, dan sosial, dan menekankan perlunya intervensi holistik untuk mencegah kekambuhan dan meningkatkan kualitas hidup pasien.References
