KOEKSISTENSI SINDROM NEFRITIK DAN NEFROTIK PADA ANAK USIA 3 TAHUN : LAPORAN KASUS

Secara global, infeksi streptokokus dan penyakit ginjal pascainfeksi masih menjadi penyebab penting morbiditas pada populasi anak, terutama di negara berkembang. Glomerulonefritis akut pascainfeksi (GNAPS) umumnya bermanifestasi sebagai sindrom nefritik klasik dengan hematuria, edema, dan hipertensi. Namun, pada sebagian kecil kasus, dapat ditemukan gambaran tumpang tindih antara sindrom nefritik dan nefrotik sehingga menimbulkan tantangan diagnostik maupun terapeutik. Koeksistensi kedua sindrom ini pada anak merupakan kondisi yang relatif jarang dan sering kali mengindikasikan proses inflamasi glomerulus yang lebih berat. Dalam laporan ini dipresentasikan seorang anak perempuan berusia 3 tahun dengan keluhan BAK merah sejak 3 hari, demam, edema wajah, serta muntah berulang. Pemeriksaan laboratorium menunjukkan hematuria masif, proteinuria signifikan, peningkatan leukosit, serta penurunan C3. USG ginjal memperlihatkan peningkatan ekogenisitas korteks yang mengarah pada proses kronis. Pasien didiagnosis dengan kombinasi GNAPS, sindrom nefritik, dan nefrotik, kemudian ditatalaksana dengan terapi suportif, kontrol tekanan darah, diuretik, antibiotik, kortikosteroid, dan direncanakan untuk biopsi ginjal. Laporan ini menegaskan pentingnya kewaspadaan klinis terhadap kemungkinan tumpang tindih sindrom nefritik dan nefrotik pada anak dengan hematuria dan edema, karena kondisi ini memiliki implikasi klinis dan prognosis yang berbeda dibandingkan presentasi tunggal.References
