TATALAKSANA NON-UNION TIBIA DENGAN ORIF (OPEN REDUCTION AND INTERNAL FIXATION) DAN MALUNION FIBULA DENGAN OSTEOTOMI : LAPORAN KASUS

Fraktur tibia dan fibula sering menimbulkan komplikasi berupa non-union dan malunion, yang dapat menyebabkan nyeri kronis, disabilitas, deformitas tungkai, dan gangguan biomekanik. Kombinasi non-union tibia dan malunion fibula jarang dilaporkan dan menimbulkan tantangan klinis dalam tatalaksana. Dilaporkan kasus seorang laki-laki berusia 60 tahun yang datang dengan keluhan nyeri menetap pada tungkai bawah kanan sejak satu tahun sebelumnya, memberat dalam satu bulan terakhir. Pasien memiliki riwayat jatuh dari sepeda motor lima tahun sebelumnya dan hanya menjalani terapi tradisional tanpa penanganan medis formal. Keluhan utama meliputi nyeri kronis yang mengganggu aktivitas, deformitas tungkai, dan pemendekan tungkai, tanpa perdarahan atau gejala sistemik. Pasien memiliki kebiasaan merokok. Pemeriksaan menunjukkan non-union tibia tengah kanan dan malunion distal fibula kanan. Pasien menjalani ORIF dan osteotomi untuk koreksi deformitas, dengan hasil pascaoperasi menunjukkan posisi implant stabil, perbaikan alignment tulang, penurunan leg length discrepancy (LLD) menjadi 1 cm, serta perbaikan nyeri dan fungsi tungkai tanpa komplikasi akut. Kasus ini menekankan pentingnya penanganan segera pada fraktur tibia dan fibula untuk mencegah malunion dan non-union.
