Abstract

Menurut perkiraan WHO, skabies mempengaruhi lebih dari 200 juta individu diseluruh dunia pada satu waktu. Meskipun skabies dapat dicegah dan diobati, penyakit ini mempengaruhi lebih dari 130 juta orang di seluruh dunia pada suatu waktu, yang menyebabkan lebih dari 1,5 juta tahun hidup dengan disabilitas di seluruh dunia. Indonesia memiliki beban skabies tertinggi di antara 195 negara yang diinvestigasi, dengan Disability Adjusted Life Year (DALYs) yang distandarisasi berdasarkan usia sebesar 153,86 per 100.000 penduduk. Crusted scabies merupakan bentuk skabies yang berat dan sangat menular. Penyakit ini terutama menyerang individu dengan sistem kekebalan tubuh yang lemah, diabetes melitus menjadi faktor predisposisi yang signifikan. Namun penyakit ini seringkali menyerupai kondisi dermatologis lain seperti psoriasis, yang menyebabkan keterlambatan diagnosis dan pengobatan yang tidak tepat. Kami melaporkan sebuah kasus crusted scabies dengan infeksi sekunder pada wanita 48 tahun yang memiliki riwayat diabetes mellitus tidak terkontrol. Pasien mengeluhkan terdapat banyak bintik kemerahan dan bercak kehitaman pada seluruh tubuh kecuali area wajah sejak 2 bulan lalu yang memberat 3 minggu terakhir hingga disertai sisik tebal berwarna putih kekuningan. Status gizi pasien termasuk gemuk tingkat berat berdasarkan klasifikasi Kemenkes RI. Pasien dirawat secara holistik bersama dengan spesialis penyakit dalam untuk mengontrol kondisi hiperglikemik yang dialami.
