DEEP VEIN THROMBOSIS PADA KOMORBID HIPERTENSI DAN GAGAL JANTUNG

Kombinasi dari deep vein thrombosis (DVT) dan pulmonary embolism merujuk pada suatu istilah yang dinamakan venous thromboembolism (VTE), adalah penyakit kardiovaskular yang umum terlihat pada pasien dewasa dan berhubungan dengan morbiditas yang tinggi. Istilah trombosis merujuk pada pembentukan massa abnormal dalam sistem vaskular. Ketika proses ini terjadi di dalam vena dalam, hal ini disebut sebagai DVT. Kami melaporkan kasus DVT pada laki-laki 54 tahun yang mengeluhkan bengkak disertai nyeri pada kaki kiri sejak 1 minggu lalu. Pemeriksaan tanda vital didapatkan hipertensi grade II. Pemeriksaan ekstremitas inferior didapatkan edema tibial kiri disertai eritema, terlihat adanya varises dari bagian tibial hingga femoral, suhu ekstremitas kiri lebih hangat dibanding kanan. Pemeriksaan laboratorium ditemukan kadar D-dimer meningkat hingga 5.548,6 ng/mL, peningkatan ureum dan kreatinin. Pemeriksaan chest x-ray didapatkan kardiomegali dan pemeriksaan venous ultrasonography (USG) didapatkan gambaran trombus vena dalam pada femoral vein, common femoral vein, poplitea vein, proximal anterior tibial vein sinistra, disertai soft tissue swelling regio femur pedis sinistra. Selain itu kami juga memberikan diskusi tentang diagnosis kondisi tersebut menggunakan algoritma diagnostik yang menggabungkan penilaian klinis, pengujian D-dimer, dan studi pencitraan. Compression ultrasonography (CUS) adalah modalitas diagnostik yang mudah diakses sehingga banyak dilakukan untuk mendukung diagnosis DVT. Kriteria yang dapat digunakan untuk menegakan diagnosis adalah Skor Wells. Tatalaksana dari DVT adalah dengan antikoagulan hingga mencapai profil koagulasi tertarget.References
