HEART FAILURE WITH PRESERVED EJECTION FRACTION (HFpEF) AKIBAT VALVULAR HEART DISEASE PADA USIA MUDA

Gagal jantung atau heart failure (HF) adalah kondisi yang sering mengenai populasi lansia, namun jarang pada usia muda. Meskipun jarang, dampaknya mungkin lebih signifikan pada produktivitas. Valvular heart disease (VHD) adalah salah satu penyebab HF, terutama HF with preserved ejection fraction (HFpEF). VHD yang mendasari dapat disebabkan oleh berbagai etiologi, Kami melaporkan kasus HF pada seorang wanita usia 22 tahun yang disebabkan oleh VHD. Pemeriksaan ekokardiografi menunjukkan adanya dilatasi keempat ruang jantung dengan hipertrofi ventrikel kiri eksentrik, regurgitasi mitral berat et causa flail posterior mitral leaflet, regurgitasi tricuspid berat dan probablitias hipertensi pulmonal tinggi. Karena presentasinya yang sudah lanjut, penyebab yang mendasari sulit untuk ditemukan, dicurigai merupakan kondisi kongenital mempertimbangkan usia pasien, ataupun penyakit jantung rematik (PJR) karena endemisitas infeksi GAS di negara berkembang. Pasien menjalani stabilisasi dan dirujuk untuk pengobatan definitif karenahanya terdapat sedikit bukti mengenai terapi medis yang efektif pada MR kronis yang berat, sehingga pembedahan tetap menjadi landasan pengobatan.References
