LAPORAN KASUS : BULLOUS SYSTEMIC LUPUS ERYTHEMATHOSUS

Lupus eritematosus sistemik bulosa (BSLE) adalah penyakit lupus eritematosus sistemik yang jarang terjadi, biasanya menyerang wanita dengan insiden tertinggi pada keturunan Afrika. Erupsi vesikulobulosa akut pada BSLE umumnya cenderung mempengaruhi area yang terpapar sinar matahari. Bula dapat timbul dari makula eritematosa, plak inflamasi, atau kulit yang sebelumnya normal. Keterlibatan organ dalam terjadi pada 90% penderita. Penatalaksanaan lini pertama dengan dapson harus dipertimbangkan pada penyakit ringan dengan lupus eritematosus sistemik yang stabil. Dalam beberapa kasus BSLE yang sulit disembuhkan yang tidak responsif terhadap dapson, glukokortikoid, atau imunosupresan hemat steroid. Kami memeriksa kasus seorang pasien perempuan usia 54 tahun datang dengan keluhan muncul bercak kemerahan dengan kulit yang mengelupas disertai luka yang berdarah serta nyeri pada seluruh tubuhnya sejak 4 bulan. Pemeriksaan status lokalis didapatkan lesi dengan distribusi universal, regio di seluruh tubuh , berjumlah multiple dengan ukuran lenticular hingga plakat, bentuk lesi tidak teratur, berbatas tegas, lesi berwarna hiperpigmentosa dan eritematosa, dan lesi basah. Efloresensi primer lesi berupa makula eritematosa dan hiperpigmentosa, dan efloresensi sekunder terdapat skuama, krusta, erosi, ekskoriasi. Pemeriksaan biopsi kulit didapatkan reaksi vesicobulosa dengan pembentukan blister subepidermal yang berisi sel limfosit, neutrophil, eosinophil dan tampak dilatasi pembuluh darah disertai infiltrate sel radang limfosit dan neutrophil di perivascular.References
