ANESTESI EPIDURAL TORAKAL (TEA) PADA OPERASI MODIFIED RADICAL MASTECTOMY (MRM)

Selama ini operasi Modified Radical Mastectomy (MRM) biasa dilakukan dengan anestesi umum. Di rumah sakit kami Thoracal Epidural Anesthesia (TEA) biasa digunakan pada gastrectomy atau thoracotomy,namun laporan penggunaan pada MRM sangat jarang. Laporan kasus ini adalah 3 kasus pasien yang direncanakan MRM dengan teknik TEA. Insersi kateter di Th3-Th4 atau Th4-Th5 secara paramedian approach dengan teknik LOR. Tes dose Lidokain 1% dan adrenalin sebanyak 3 ml dilanjutkan dosis awal 5-10 ml Bupivacain isobaric 3.75% hingga target blok antara C5–Th7. Monitoring hemodinamik selama operasi. Hasil temuan Selama operasi ketiga pasien menunjukkan hemodinamik stabil dan selesai tanpa penyulit lalu pasien ditransfer ke ruang pemulihan. Tiga hari paska bedah diberikan Bupivakain 0.125% plus ajuvan fentanyl melalui epidural.. Evaluasi skala nyeri NRS sekitar 2-3. Kesimpulan dari kasus ini bahwa Teknik TEA pada operasi MRM ideal digunakan selain anestesi umum yang rutin dilakukan. TEA sangat baik dalam pengelolaan nyeri akut paska bedahReferences
