Keterampilan Perawatan Keluarga Memediasi Pengetahuan dan Risiko Ulkus Tekanan Pasca Stroke

Abstrak Ulkus tekan merupakan komplikasi umum pada pasien pasca-stroke akibat keterbatasan mobilitas. Pengasuh keluarga memainkan peran penting dalam pencegahan; namun, pengetahuan saja mungkin tidak dapat diterjemahkan menjadi praktik yang efektif tanpa keterampilan pengasuhan yang memadai. Studi ini bertujuan untuk meneliti peran mediasi keterampilan pengasuhan keluarga dalam hubungan antara pengetahuan pengasuh dan risiko ulkus tekan pada pasien pasca-stroke. Sebuah studi kuantitatif potong lintang dilakukan yang melibatkan 60 pasien pasca-stroke dan pengasuh keluarga utama mereka di lingkungan rumah sakit. Pengetahuan pengasuh diukur menggunakan kuesioner yang diadaptasi dari Beeckman dkk. (2011), keterampilan pengasuhan dinilai menggunakan instrumen Intervensi Protokol Pencegahan Ulkus Tekan, dan risiko ulkus tekan dievaluasi menggunakan Skala Braden. Analisis regresi linier digunakan untuk menguji hubungan langsung dan mediasi. Hasil menunjukkan hubungan yang signifikan antara pengetahuan pengasuh dan keterampilan pengasuhan (p = 0,007) dan antara keterampilan pengasuhan dan penurunan risiko ulkus tekan (p = 0,033). Tidak ditemukan hubungan langsung antara pengetahuan pengasuh dan risiko ulkus tekan (p = 0,849). Temuan ini menunjukkan bahwa keterampilan merawat berperan sebagai perantara hubungan antara pengetahuan dan risiko ulkus tekan. Oleh karena itu, pelatihan berbasis keterampilan bagi perawat keluarga direkomendasikan untuk secara efektif mengurangi risiko ulkus tekan pada pasien pasca stroke. Family caregivers play an essential role in prevention; however, knowledge alone may not translate into effective practice without adequate caregiving skills. This study aimed to examine the mediating role of family caregiving skills in the relationship between caregiver knowledge and pressure ulcer risk among post-stroke patients. A quantitative cross-sectional study was conducted involving 60 post-stroke patients and their primary family caregivers in a hospital setting. Caregiver knowledge was measured using a questionnaire adapted from Beeckman et al. (2011), caregiving skills were assessed using the Pressure Ulcer Prevention Protocol Intervention instrument, and pressure ulcer risk was evaluated using the Braden Scale. Linear regression analysis was used to test direct and mediating relationships. The results showed a significant association between caregiver knowledge and caregiving skills (p = 0.007) and between caregiving skills and reduced pressure ulcer risk (p = 0.033). No direct relationship was found between caregiver knowledge and pressure ulcer risk (p = 0.849). These findings indicate that caregiving skills mediate the relationship between knowledge and pressure ulcer risk. Therefore, skills-based training for family caregivers is recommended to effectively reduce pressure ulcer risk in post-stroke patients.
