LAPORAN KASUS : INVERTED PAPILOMA SINONASAL PADA PEROKOK

Inverted papilloma sinonasal merupakan tumor jinak epitel yang bersifat lokal agresif, memiliki kecenderungan kambuh, serta berpotensi mengalami transformasi malignan. Penyebab pasti inverted papilloma belum diketahui, namun diduga karena beberapa faktor, yaitu merokok, alergi atau paparan pekerjaan tertentu. Tujuan laporan kasus ini adalah membahas seorang pasien laki-laki berusia 46 tahun yang datang dengan keluhan hidung tersumbat unilateral kronis, disertai penurunan penciuman, rinorea bening, dan nyeri kepala. Riwayat merokok berat ditemukan sebagai salah satu faktor risiko yang berperan. Pemeriksaan fisik menunjukkan adanya massa polipoid pada kavum nasi, sedangkan pemeriksaan penunjang CT-scan memperlihatkan massa sinonasal tanpa destruksi tulang. Pemeriksaan klinis dengan rinoskopi anterior, secara makroskopik tampak tumor ini hampir serupa dengan polip hidung biasa. Diagnosis ditegakkan melalui pemeriksaan histopatologis hasil biopsi yang menunjukkan gambaran inverted papilloma tanpa tanda keganasan. Pemeriksaan radiologi yang dilakukan adalah CT Scan Sinus dan MRI yang bertujuan untuk mengetahui perluasan dan lokasi tumor. Penatalaksanaan dilakukan dengan tindakan bedah eksisi luas radikal disertai pemberian terapi antibiotik dan suportif sebelum serta setelah operasi. Tatalaksana ini bertujuan untuk mengangkat tumor secara menyeluruh guna menurunkan risiko kekambuhan. Kasus ini menegaskan pentingnya pendekatan diagnostik yang komprehensif pada pasien dengan obstruksi hidung unilateral kronis serta perlunya pengelolaan bedah yang adekuat dan pemantauan jangka panjang untuk mencegah rekurensi dan mendeteksi kemungkinan transformasi malignan secara dini.References
