KORELASI LDL DAN MIKROALBUMINURIA PADA PASIEN DIABETES MELLITUS TIPE 2

Mikroalbuminuria merupakan penanda dini kerusakan ginjal diabetik pada pasien Diabetes Melitus (DM) tipe 2 dan berhubungan dengan peningkatan risiko penyakit ginjal kronis. Dislipidemia, terutama peningkatan kadar low density lipoprotein (LDL), diduga berperan dalam kerusakan glomerulus, namun bukti hubungan langsung LDL dengan mikroalbuminuria di layanan primer Indonesia masih terbatas. Penelitian ini bertujuan menilai korelasi antara kadar LDL dan mikroalbumin urin pada pasien DM tipe 2 peserta Program Pengelolaan Penyakit Kronis (Prolanis) di Puskesmas Kedaton. Desain penelitian menggunakan analitik observasional potong lintang pada 87 pasien yang memenuhi kriteria inklusi. Data LDL (mg/dL) dan mikroalbumin urin (mg/L) diperoleh dari rekam medis dan pemeriksaan laboratorium. Uji normalitas Kolmogorov–Smirnov menunjukkan distribusi normal pada LDL dan tidak normal pada mikroalbumin, sehingga analisis korelasi menggunakan uji Spearman. Hasil menunjukkan korelasi positif sangat lemah antara LDL dan mikroalbumin urin (r = 0,152; p = 0,160), sehingga tidak terdapat hubungan bermakna secara statistik. Temuan ini mengindikasikan bahwa kadar LDL total bukan prediktor utama mikroalbuminuria pada pasien DM tipe 2 di layanan primer. Faktor risiko lain, seperti kontrol glikemik, tekanan darah, dan durasi penyakit, tetap perlu menjadi fokus utama dalam pencegahan dan deteksi dini penyakit ginjal diabetik. Penelitian ini menekankan pentingnya pendekatan multifaktor untuk menurunkan risiko komplikasi ginjal pada pasien DM tipe 2 di setting primer.References
