Discharge Planning merupakan salah satu komponen penting dalam praktik keperawatan yang bertujuan memastikan pasien dan keluarga siap melanjutkan perawatan di rumah setelah keluar dari rumah sakit. Pada pasien dengan Congestive Heart Failure (CHF), perencanaan pulang yang kurang optimal dapat meningkatkan risiko kekambuhan, komplikasi, hingga rehospitalisasi .Studi ini menggunakan pendekatan deskriptif dengan pelaksanaan Discharge Planning di ruang CVBCRumah Sakit Dr R.D Kandou Manado.Hasil penelitian menunjukkan perawat memiliki pemahaman yang lebih baik terkait alur discharge planning. Tersedianya media edukasi berupa leaflet mempermudah pasien dan keluarga dalam memahami langkah perawatan CHF di rumah, seperti manajemen obat, pola makan, aktivitas, dan pemantauan gejala.Kesimpulan Discharge Planning yang disertai dengan media leaflet memberikan kontribusi positif terhadap peningkatan kualitas edukasi pada pasien CHF.References
