EVALUASI PROGRAM HIPERTENSI DI PUSKESMAS SIRAIT, SAMOSIR: STUDI LOGIC MODEL TAHUN 2022-2024

ABSTRAK Pendahuluan: Hipertensi merupakan masalah kesehatan global yang dikenal sebagai silent killer karena gejalanya sering tidak disadari, namun berisiko menimbulkan komplikasi berat. Di Indonesia, prevalensi hipertensi meningkat signifikan setiap tahun, termasuk di Kabupaten Samosir. Metode: Penelitian ini bertujuan mengevaluasi pelaksanaan program hipertensi di Puskesmas Sirait menggunakan pendekatan logic model, menilai efektivitas, mengidentifikasi kendala, serta memberikan rekomendasi perbaikan. Penelitian ini menggunakan desain evaluasi deskriptif dengan pendekatan campuran. Data dikumpulkan dari laporan tahunan program hipertensi tahun 2022–2024, wawancara kepada penanggung jawab program, serta observasi langsung. Analisis data dilakukan secara deskriptif kuantitatif dan kualitatif. Hasil: Penelitian menunjukkan prevalensi hipertensi di wilayah kerja Puskesmas Sirait mencapai 15–29%, namun cakupan layanan hanya 15,26–25,40%. Terdapat kendala dalam ketersediaan SDM, alokasi anggaran, keterbatasan pelatihan, serta belum optimalnya distribusi dan edukasi. Tidak terdapat data pasti mengenai kepatuhan terapi, perubahan gaya hidup, dan penurunan tekanan darah. Outcome dan impact program belum menunjukkan hasil yang memuaskan, dengan prevalensi dan komplikasi yang terus meningkat. Simpulan: Penelitian ini menunjukkan bahwa pelaksanaan program hipertensi di Puskesmas Sirait telah berjalan, namun belum efektif dalam menurunkan prevalensi dan meningkatkan capaian layanan. Diperlukan evaluasi berkala, pendataan lebih rinci, peningkatan SDM, serta integrasi pelayanan dan edukasi berbasis komunitas untuk meningkatkan efektivitas program. Kata kunci: Evaluasi program, hipertensi, model logika ABSTRACT Background: Hypertension is a global health issue known as a silent killer due to its often unnoticed symptoms, which can lead to severe complications. In Indonesia, the prevalence of hypertension has significantly increased annually, including in Samosir Regency. Method: This study aims to evaluate the implementation of the hypertension program at Puskesmas Sirait using a logic model approach, assess its effectiveness, identify obstacles, and provide recommendations for improvement. This research employed a descriptive evaluation design with a mixed-method approach. Data were collected from annual hypertension program reports from 2022 to 2024, interviews with the program officer, and direct observations. Data analysis was conducted descriptively using both quantitative and qualitative methods. Results: Showed that hypertension prevalence in the working area of Puskesmas Sirait reached 15–29%, but service coverage only ranged from 15.26–25.40%. Constraints were identified in human resources, budget allocation, limited training, and suboptimal distribution and education efforts. No specific data were available regarding treatment adherence, lifestyle changes, or blood pressure reduction. Program outcomes and impacts have not yielded satisfactory results, with both prevalence and complications continuing to increase. Conclusion: The hypertension program at Puskesmas Sirait has been implemented, it has not yet been effective in reducing prevalence and improving service coverage. Periodic evaluations, detailed data recording, increased human resources, and community-based integrated services and education are needed to improve program effectiveness.
