HERPES ZOSTER OFTALMIKUS PADA PASIEN DENGAN GAGAL GINJAL KRONIS: SEBUAH LAPORAN KASUS

Pendahuluan: Herpes zoster merupakan penyakit neurokutan dengan manifestasi erupsi vesikular berkelompok dengan dasar eritematosa disertai nyeri radikular unilateral yang umumnya terbatas di suatu dermatom. Herpes zoster merupakan manifestasi reaktivasi infeksi laten endogen virus varicella zoster (VZV) di dalam neuron ganglion sensoris radiks dorsalis, ganglion saraf kranialis, atau ganglion saraf otonom yang menyebar ke jaringan saraf dan kulit dengan segmen yang sama. Insiden dan keparahan penyakitnya meningkat dengan bertambahnya usia. Laporan Kasus: Seorang laki-laki, 53 tahun datang dengan keluhan lenting pada wajah bagian kiri sejak 3 hari yang terasa nyeri dan panas. Lenting tersebut awalnya muncul di atas bibir kiri, kemudian menyebar ke hidung dan kelopak mata kiri. Lenting berwarna kemerahan dan semakin lama semakin banyak. Pasien juga mengeluhkan mata kiri menjadi berair dan kelopak mata bengkak sehingga sulit untuk membuka mata. Pasien tidak mengeluhkan adanya gangguan penglihatan. Dua hari sebelum muncul lenting tersebut pasien mengeluhkan demam dan pusing. Pasien belum pernah mengalami keluhan tersebut sebelumnya. Pasien memiliki riwayat gagal ginjal kronis dan rutin hemodialisa. Pada pemeriksaan fisik ditemukan lesi pada regio wajah kiri dengan efloresensi berupa vesikel, pustul, dan krusta, berukuran 0,2-0,4 cm, bentuk bulat dengan susunan berkelompok, penyebaran regional, 1 dermatom, unilateral, jumlah multipel, dan berwarna eritema. Pemeriksaan laboratorium menunjukkan adanya anemia dan peningkatan ureum dan kreatinin. Pasien diberikan terapi antiviral, antiinflamasi, analgetik, dan antibiotik topikal. Kesimpulan: Pasien herpes zoster dengan gagal ginjal kronis memerlukan perhatian khusus terutama pada penyesuaian dosis terapi.References
