KORELASI TEKANAN DARAH DENGAN LAJU FILTRASI GLOMERULUS PADA PASIEN HIPERTENSI DI RS HERMINA KEMAYORAN

Hipertensi merupakan penyakit tidak menular kronis yang menjadi faktor risiko utama kerusakan organ target, termasuk organ ginjal. Tekanan darah yang tidak terkontrol dapat menyebabkan gangguan struktural dan fungsional pada pembuluh darah ginjal, sehingga berdampak pada penurunan laju filtrasi glomerulus (LFG), salah satu indikator penting fungsi ginjal. Penurunan LFG pada penderita hipertensi dapat terjadi lebih cepat dibandingkan individu normotensi. Mean Arterial Blood Pressure (MABP) dianggap mencerminkan perfusi organ, termasuk ginjal, meskipun bukti ilmiah mengenai hubungannya dengan LFG masih terbatas. Penelitian ini bertujuan untuk mengetahui hubungan antara tekanan darah sistolik (TDS), tekanan darah diastolik (TDD), dan MABP dengan LFG pada pasien hipertensi. Penelitian ini menggunakan desain analitik observasional dengan pendekatan cross-sectional (potong lintang). Populasi penelitian adalah seluruh pasien hipertensi di Rumah Sakit Hermina Kemayoran, Jakarta Pusat. Sampel yang diambil sebanyak 70 pasien dengan memenuhi kriteria inklusi dan eksklusi. Metode sampling menggunakan teknik Consecutive Sampling. Variabel yang dianalisis meliputi TDS, TDD, MABP, dan LFG, dengan perhitungan LFG menggunakan rumus CKD-EPI 2021 berdasarkan kadar kreatinin serum (mg/dL). Data diperoleh dari rekam medis dan dianalisis menggunakan SPSS. Uji korelasi Spearman digunakan untuk menganalisis hubungan TDS dan TDD dengan LFG, sedangkan uji korelasi Pearson untuk menganalisis hubungan untuk MABP dengan LFG. Hasil menunjukkan korelasi negatif yang signifikan antara TDS dan LFG (r = -0,453; p = 0,001), namun tidak ditemukan korelasi yang signifikan pada TDD dan LFG (r = -0.210; p = 0.080) maupun antara MABP dan LFG (r = -0.003; p = 0.977). Disimpulkan bahwa peningkatan TDS berhubungan dengan penurunan LFG pada pasien hipertensi.References
