PENERAPAN HEAD UP 30˚ TERHADAP PERFUSI SEREBRAL DAN HEMODINAMIK PASIEN POST-CRANIECTOMY HARI KE-7 DI RUANGAN INTENSIVE CARE UNIT RSUD TEMANGGUNG

Cedera kepala berat sering menyebabkan gangguan perfusi serebral dan ketidakstabilan hemodinamik, terutama setelah tindakan dekompresi seperti kraniektomi. Posisi head-up 30° kerap digunakan dalam manajemen neurointensif untuk membantu menurunkan tekanan intrakranial (TIK) dan memperbaiki aliran darah otak, namun efektivitasnya pada hari ke-7 pasca operasi masih perlu dikaji lebih lanjut. Tujuan penelitian ini ingin mengetahui pengaruh posisi head-up 30° terhadap perfusi serebral dan parameter hemodinamik pada pasien pasca kraniektomi akibat cedera kepala berat di hari ke-7 perawatan di ruangan ICU RSUD Temanggung. Penelitian ini menggunakan pendekatan studi kasus dengan menerapkan proses keperawatan mulai dari tahap pengkajian hingga evaluasi pada pasien dengan cedera kepala berat pasca kraniektomi. Data dikumpulkan melalui observasi klinis dan pencatatan parameter hemodinamik serta tanda perfusi serebral selama penerapan posisi head-up 30°. Sebelum intervensi, ditemukan fluktuasi tekanan darah, nadi, dan penurunan saturasi oksigen hingga 80%. Setelah penerapan posisi head-up 30°, terjadi peningkatan saturasi oksigen (hingga 100%), stabilisasi denyut nadi dan frekuensi pernapasan, serta perbaikan skor Glasgow Coma Scale (GCS) dari E4M3VETT menjadi E4M5VTT. Meskipun tekanan darah sedikit menurun, hemodinamik pasien tetap dalam batas normal. Penerapan posisi head-up 30° pada pasien pasca kraniektomi hari ke-7 menunjukkan dampak positif terhadap perfusi serebral dan stabilitas hemodinamik. Intervensi ini dapat direkomendasikan dalam praktik keperawatan neurointensif untuk mendukung pemulihan fungsi otak.References
