ABSTRAK Angka Kematian Ibu (AKI) merupakan salah satu indikator kesehatan suatu bangsa. Kematian ibu merupakan kematian seorang wanita yang dapat disebabkan pada saat kondisi hamil atau menjelang 42 hari setelah persalinan. Hasil pencatatan dan pelaporan dari pelayanan kesehatan yang ada diketahui bahwa AKB di Provinsi Kepulauan Riau tahun 2018 ialah sebesar 7.01 per 1.000 kelahiran hidup. Angka Kematian Ibu (AKI) Provinsi Kepulauan Riau Tahun 2018 yaitu sebesar 120 per 100.000 kelahiran hidup. Tujuan dari penyusunan Laporan Tugas Akhir ini adalah melakukan asuhan kebidanan pada masa kehamilan, persalinan, nifas, bayi baru lahir, dan keluarga berencana pada Ny. K usia 31 tahun G3P2A0 di BPM Atlantika Amd.Keb Tangki Seribu Kota Batam. Metode yang digunakan penulis adalah wawancara, observasi, pemeriksaan fisik. Melalui pendekatan manajemen asuhan kebidanan Continuity Of care di BPM Atlantika, Amd.Keb diberikan pada kunjungan kehamilan dilakukan sebanyak 2 kali, ibu bersalin 1 kali, nifas 3 kali, bayi baru lahir 3 kali, dan keluarga berencana 1 kali. Proses persalinan pada Ny. K berlangsung secara normal. Asuhan nifas dan bayi baru lahir dalam keadaan baik. Pada asuhan keluarga berencana berjalan dengan baik ibu sebagai calon akseptor Kontrasepsi 3 Bulan. Kata kunci: Bayi Baru Lahir, Kehamilan, Keluarga Berencana, Nifas, Persalinan ABSTRACT Maternal Mortality Rate (MMR) is one indicator of a nation's health. Maternal death is the death of a woman that can be caused during pregnancy or before 42 days after delivery. The results of recording and reporting from existing health services show that the IMR in the Riau Islands Province in 2018 was 7.01 per 1,000 live births. The Maternal Mortality Rate (MMR) of Riau Islands Province in 2018 is 120 per 100,000 live births.The purpose of the preparation of this Final Project is to provide midwifery care during pregnancy, childbirth, postpartum, newborn, and family planning to Ny. K, 31 years old, G3P2A0 at BPM Atlantika Amd.Keb Tangki Seribu, Batam City. The method used by the author is interviews, observation, physical examination. Through the management approach of midwifery care Continuity Of care at BPM Atlantica, Amd.Keb is given at pregnancy visits carried out 2 times, mothers give birth 1 time, postpartum 3 times, newborns 3 times, and family planning 1 time. The process of giving birth to Mrs. K proceeds normally. Postpartum care and newborns are in good condition. In family planning care, the mother is a prospective acceptor of 3 Months Contraception.
