PERBANDINGAN EFEKTIVITAS TERAPI METFORMIN DAN DIET RENDAH LEMAK/KALORI TERHADAP PASIEN DM TIPE II : LITERATURE REVIEW

Diabetes mellitus tipe 2 (DMT2) sebagai penyakit metabolik tidak menular yang ditandai dengan kadar glukosa darah tinggi, hasil dari gangguan sekresi insulin, resistensi insulin atau kombinasi keduanya. Pencarian artikel menggunakan keyword “Metformin AND Probiotic AND Type 2 diabetes mellitus” pada pubmed ditemukan 32 artikel, Cochrane 17 artikel, dan JSTOR 27 artikel. Artikel dari database yang dipakai dilakukan screening duplikasi sehingga didapatkan 30 jurnal dari database Pubmed dan Cochrane. Artikel diseleksi kembali dengan filter eksklusi “paid text dan more than 5 years,” sehingga didapatkan 16 artikel dari Pubmed dan Cochrane, sedangkan pada JSTOR tidak ada jurnal yang lulus seleksi. Penapisan yang dilakukan menggunakan kriteria eksklusi “Other language, Unlock abstract, Systematic review, Meta-analysis, Case report, Proposal” sehingga diperoleh 6 artikel. Terakhir dilakukan screening dengan kriteria eksklusi “Type 1 Diabetes Mellitus dan Other than mix intestinal microbiota” sehingga diperoleh 3 artikel yang relevan dengan masalah yaitu randomized controlled trial (RCT). Berdasarkan penelitian Palacios et.al., intervensi subjek selama 12 minggu yang menggunakan kombinasi terapi probiotik dan metformin menunjukkan terjadinya peningkatan SCFA (berperan penting terhadap kesehatan saluran cerna) dan butirat. Untuk penggunaan terapi probiotik saja akan meningkatkan produksi SCFA sedangkan penggunaan terapi metformin saja tidak ada pengaruh terhadap kadar SCFA dan kadar butirat (Butirat telah terbukti menjaga integritas barier usus. Studi ini memunjukkan peningkatan bakteri probiotik yang cenderung menurunkan bakteri pro-inflamasi.References
