OPTIMALISASI IMPLEMENTASI PENCEGAHAN RISIKO JATUH PADA PASIEN MENGGUNAKAN TIGA TAHAPAN PERUBAHAN LEWINS

Pencegahan risiko jatuh merupakan hal penting dalam proses penerapan keselamatan pasien di rumah sakit. Akibat jika terjadi cidera dari kejadian jatuh akan menambah angka kesakitan pasien, memperpanjang lama rawat serta akan merugikan pasien dan RS. Meskipun pravalensi cidera akibat jatuh tinggi, kejadian ini sangat bisa dicegah dengan menggunakan asesmen awal risiko jatuh pada pasien. Dimana penelitian menunjukkan bahwa hampir 33% pasien jatuh pada saat rawat inap dapat dicegah dengan pengelolaan faktor ancaman jatuh dari pasaien serta mengelola longkungan dan proses perawatan. Laporan kasus ini bertujuan untuk mendeskripsikan pelaksanaan perubahan terencana dalam optimalisasi implementasi pencegahan risiko jatuh. Metode yang digunakan adalah laporan kasus. Pengumpulan data menggunakan wawancara, kuesioner dan observasi lapangan. Hasil eksplorasi data kemudian dianalisis dengan menggunakan strength, weakness, opportunity, threats (SWOT) dan diagram fishbone, kemudian merumuskan plan of action (PoA), dilanjutkan dengan implementasi dan evaluasi. Hasil implementasi belum optimal dilakukan karena perlu dilakukan evaluasi panduan risiko jatuh , program supervisi secara rutin serta dilakukan audit klinis atau diskusi refleksi kasus terkait dengan pencegahan risiko jatuh. Pendekatan teori perubahan Lewin dapat dilanjutkan oleh pihak manajemen RS sebagai bentuk pengembangan pelayanan yang berkualitas.References
